The Care Planning Training Support Programme (TSP)
National Care Planning Training provides a bespoke core training programme for healthcare professionals which equips them with the practical skills and tools required to implement Year of Care and Care Planning in practice across an organisation.
The Train the Trainers programme enables new organisations to identify and train local individuals to deliver quality assured training themselves. This provides a sustainable method of rolling out care planning across a health community, and provides support to local practice as they work to make care planning routine.
All new sites involved in the core training programme will have access to a secure website of resources which will not only support the delivery of training, but also the implementation of care planning itself. This includes a range of training programmes to support care planning, IT guidance, a Practice Pack with information for administrative staff and practice managers, an evaluation framework and patient resources (this is not an exhaustive list).

- Taster sessions - Preparing for Care Planning
- Core Care Planning Training
- Healthcare Assistant Training
- Care Planning Awareness Raising
- District Nurse Training in Care Planning
- Train the Trainer and Quality Assurance Programme
- Long Term Conditions (LTCs) Care Planning Training for practice teams
- Training for integrated community teams - working with people with LTCs risk assessed as in the top 7% of the practice population as likely to use unscheduled care – (under development)
- Extended Consultation Skills for clinical staff (under development)
- A programme to set up local mentoring and support to build confidence, embed and transfer skills and attitudes across the local community.
