Needs assessment for self management support

The second component of the YOC programme was to use care planning as a means of identifying the services people would value to assist them to achieve their goals and carry out their action plans over the year following the care planning consultation itself. The potential for commissioners was both to provide a far more tailored set of therapeutic and support services, and also more choice. This could involve a much wider array of third sector and independent providers of Support for Self Management.

The pilot sites:

  • identified that effective micro to macro commissioning will depend on available of IT systems for care planning intimately embedded in the Electronic Health record (EHR) and linked with third sector provider systems
  • adopted other approaches to local needs assessment so that an wider menu of care could be provided as care planning was introduced 
  • Social marketing was useful to identify the range and volume of services need, but group classification must not artificially constrain choice for individuals.
  • Stratifying the population along biomedical lines provided the basis for packages of care in Tower Hamlets (1.1MB).
