This document provides instructions for generating a random sample of patients to survey, guidance on capturing the relevant data fields, checks to avoid inappropriate mailings, directions for printing the questionnaires, sample covering letters (although your organisation may have its own preferred standard format) and advice on materials required.
Sending out a postal survey (PDF 73KB)
This document provides instructions on the mailing process including content of envelopes for each mailing, mail merge instructions for generation of labels, booking in returned questionnaires (this is also covered in Entering data) and sending out reminders.
