Brief description of unit

The Children’s Diabetes Team at CHFT covers the population of Calderdale and South Kirklees and has 2 hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary.  Our catchment area covers inner city to rural Pennine locations.  We have approximately 200 patients, up to 18 years, fairly evenly split between the 2 sites.  We have weekly clinics on separate sites at the 2 hospitals, as well as 3 monthly outreach clinics at Todmorden and Brighouse Health Centres, and monthly transition clinics on each hospital site.  We also have 2 Diabetes support workers, Rose Hagreen and Hayley Taylor, who support and motivate children and young people in the development of practical skills and problem-solving strategies to improve self-management of their diabetes care needs.
