Prevention and risk management
This page is aimed at commissioners and providers of diabetes care. It brings together guidance, policy, commissioning guides, links to care pathways and examples of how services can be improved for people with diabetes.
The challenge
About 2.8 million people have been diagnosed with diabetes in the UK, costing the NHS around £1 million per hour. An estimated 500,000 people have the condition but are unaware of it. It is predicted that over 4 million people will have diabetes in the UK by 2025.
Identifying people at an increased risk of Type 2 diabetes is a key strategy in the prevention of the disease. Evidence suggests that lifestyle modification can be beneficial in preventing Type 2 diabetes in adults at high risk of developing the disease.
What NHS Diabetes is doing?
NHS Diabetes and Kidney Care are working with the Department of Health to deliver NHS Health Check, a screening programme designed to help prevent heart disease, stroke, Type 2 diabetes and kidney disease. A Learning Network has been established to support the development and implementation of NHS Health Check, and aims ‘to learn from, build upon and share the learning and experience of both existing and emerging Health Check initiatives across the country’. - A commissioning guide: NHS Diabetes Commissioning Diabetes Prevention and Risk Assessment Services - July 2011 (PDF 392KB) for diabetes prevention and risk assessment has been published by NHS Diabetes. The guide includes an intervention map detailing all of the elements needed for the service as well as a contracting framework and service specification template. It supports our commissioning approach which offers a step-by-step guide in how to deliver high quality, efficient and cost effective diabetes services.
