Care Pathway: Initial Clinic Visit with Specialist Diabetes Team
Up to one hour appointment with specialist team competent and trained to discuss and advise on the risks associated with pregnancy: It is recommended that this team consists of: Diabetes Consultant, Diabetes Specialist Nurse, Dietitian, Diabetes Obstetric Consultant, Diabetes Midwife and interpreter if necessary.

Discussion
- Discussion of the risks and implications of having diabetes on pregnancy and how these risks can be reduced

Assessment
- Assessment of current glycaemic control
- Assessment of diabetes related complications
- Assessment of current medications with regard to safety in pregnancy
- Assessment of additional obstetric risks
Advice
- Advice on smoking and alcohol reduction
- Advice on diet, body weight and exercise
- Prescription of 5mg Folic acid
- Management of effective contraception and discontinuation of contraception
- Work with patient (and partner) to develop individual plan to optimise management of diabetes preconception
Signposting
- Provide contact numbers of team
- Stratify risk to inform follow-up requirements
- Signposting and fast track to other services e.g. structured education, insulin pump and weight loss programmes
- Provision of appropriate literature to take away - e.g. SAFER leaflet (PDF 591KB)
