Care Pathway: Follow Up - Optimise Glycaemic Control
Education in Diabetes Self Management:
- How and when to do blood glucose monitoring and targets required for pregnancy (NICE Guidance CG63)
- Monthly HbA1c (aiming for
- Management of hypoglycaemia (including hypoglycaemia unawareness, driving management)
- Management and avoidance of hyperglycaemia (and ketonuria in Type 1 diabetes)
- Dietary advice to keep blood sugars stable avoiding hyperglycaemia/hypoglycaemia
- Management of diabetes through pregnancy related nausea and vomiting and intercurrent illness
- Contact numbers and phone contact follow up
Consider Referral For:
- Use of Continuous Glucose Monitoring, feedback and treatment adjustment
- Commencement of insulin and ongoing support with dosage titration
- Commencement of Continuous Subcutaneous Insulin Infusion Pump
- Structured education programme (e.g. DAFNE)
