Care Pathways: Follow Up - Optimise Management of Medical Problems
May involve clinic or non face-to-face contact by telephone and email
- Ongoing management of complications (e.g. blood pressure, ischaemic heart disease, retinopathy, renal function, gastroparesis)
- Ongoing support to reduce stress and improve adherence to diabetes treatment plan
- Responsive service to ensure timely changes are made to optimise diabetes management quickly
Consider Referral For:
- Psychological / liaison psychiatry support
- Nephrologist, cardiologist, opthalmologist for management of advanced diabetes complications
